- Please click on the application link below.
- Click the download button to begin the application process.
- Please complete the form as required.
- If your information is correct and you wish to submit your application, please click the Submit by Email link which will appear as an option at the bottom of the form.
- Depending on which version of Acrobat Reader you're using you may have the ability of submitting your application by email to our server online, or you may be directed to fill out your email address to complete submitting of your application.
- If your Acrobat Reader version does not allow you to submit the application online you can upgrade to the latest version of Acrobat Reader Free, or you can also "Save As" your application to your computer and email it to our DLC Coordinator as an attachment to cims@theniagara.org
- You may also "Print" your application and fax it to 910.423.3769
- Please note that there is a $25 NON REFUNDABLE FEE due upon submitting your application, you will be notify by the the DLC Coordinator on how to make this payment.
